Open-door vs closed-door resignations

An open-door resignation signals that you value the relationship and would consider returning, covering shifts, or collaborating in the future. This works well if you're leaving for training, relocating temporarily, or testing a different care model but respect your current employer.

A closed-door resignation is a clean break—no ambiguity, no lingering obligations. You're burned out, moving to a competing group, or the environment was toxic. You're professional but final.

For physicians, the choice often hinges on whether you want to maintain hospital privileges, referral pathways, or locums opportunities. Medicine is referral-based and credentialing is slow—burning a bridge can narrow your options for years.

Industry handover notes for Physicians

  • Patient panel documentation: Create a summary for each high-acuity or chronic patient, including treatment history, pending referrals, and upcoming follow-ups. Don't assume the EMR tells the whole story.
  • On-call and hospital privileges: Notify the medical staff office of your resignation date so your call schedule can be reassigned and privileges terminated appropriately.
  • Referral relationships: Inform key referring providers (PCPs, specialists) of your departure and introduce them to your successor if possible. Referral networks are trust-based and take years to build.
  • Credentialing materials: Confirm that your employer has your updated CV, board certifications, and DEA/license numbers for reference checks. Future employers will verify your tenure and standing.

What to do BEFORE you submit the letter

Physicians have complex employment contracts, credentialing timelines, and malpractice tail coverage to navigate. Submitting your letter without preparation can cost you months of income or saddle you with unexpected liability.

Confirm your new offer in writing. Don't resign until you have a signed offer letter with a start date, compensation details, and malpractice coverage terms. Verbal offers fall through, especially in hospital systems with budget cycles.

Review your employment contract. Check your notice period (usually 60–90 days for physicians), non-compete radius and duration, and tail coverage obligations. Some contracts require you to pay for your own tail if you resign; others cover it if you're terminated. That difference can be $15K–$50K.

Lock in your start date with credentialing in mind. Hospital credentialing takes 90–120 days. If your new role requires privileges, make sure your resignation timeline and new employer's credentialing process don't leave you with a gap in income.

Take screenshots of your patient panel and key documents. You won't have EMR access after your final day. Export or photograph summary lists of your patients, referral sources, and clinical protocols you developed—you may need them for continuity or future reference.
