MDS COORDINATOR Full Time

MDS Coordinator role requiring an RN or LPN to monitor, evaluate and manage resident care plans; use the resident management system and assessments to develop goals for resident improvement; chair the interdisciplinary care planning team; coordinate Medicare/Medicaid eligibility and reimbursements; ensure regulatory compliance (CMS, DHEC, HIPAA) and accurate medical records, submit MDS data to CMS, and prepare quarterly quality indicator reports; collaborate with Nursing, Rehabilitation, and Business Office to ensure reimbursement processes and effective care coordination.
